Key Focus Areas
In this final session titled “Preparing for Life with Dementia,” Joelle will cover essential topics including:
  • Advance Directives: Understanding the importance of legal documents that specify your wishes regarding medical treatment if you become unable to communicate them yourself.
  • Physician Orders for Life-Sustaining Treatment (POLST): Learning about the POLST form and how it ensures medical professionals follow your healthcare preferences during emergencies.
  • Supportive Community Building: Fostering a network of support for individuals and caregivers, empowering them with the necessary resources and knowledge to manage the challenges posed by dementia.
This structured program aims to provide a comprehensive framework that helps families make informed decisions about future care needs, fostering a safe and supportive environment for those affected by dementia.
